A group that participates in the ACO REACH Model is referred to as a REACH ACO. Doctors and other health care providers choose to participate in a REACH ACO because they’re committed to providing an improved care experience. They may also be rewarded financially for offering more coordinated care while keeping heath care costs low.
Doctors in REACH ACOs may also have greater access to the expertise, staff, and technologies they need to make sure care is coordinated across all the places patients get services.
Medicare benefits are unchanged for beneficiaries, who still may go to any doctor, hospital or other healthcare provider that accepts Medicare.
The goal of the REACH ACO is to support doctors in caring for their patients by making sure they have the most up-to-date information about their health and care. For Beneficiaries, this coordination could mean less paperwork to fill out at the doctor’s office, avoiding unnecessary tests, and more awareness of treatment options for various health conditions.
By working together, doctors can do more to monitor patient health and make sure they receive the highest quality care.
